Meaford Long Term Care Centre is a 77 bed Home, licensed and inspected by the Ministry of Health & Long Term Care. The Home is accredited by the Canadian Council on Health Services Accreditation and is an active member of the Ontario Long Term Care Association.

As a Home in a small community, we enjoy the benefits of long term staff and volunteers that provide consistent care and services to our Residents.

Privacy of Health Information Policy Statement

Meaford Long Term Care Centre recognizes the individual’s right to privacy in relation to their health information. While an individual does not have the property right to their record, they should have the right of protection of all information contained therein.

You have a right to know how we collect, use and disclose personal health information. You have the right to expect that, to the best of our ability, personal health information held by Meaford Long Term Care Centre remains accurate, confidential and secure.

Meaford Long Term Care Centre is committed to maintaining the confidentiality and security of personal health information and we have implemented new practices to further safeguard your information.  These new practices are aligned with the Ontario’s Ministry of Health and Long Term Care Personal Health Information Protection Act (PHIPA) and with the Canadian Standards Model Code for the Protection of Personal Information ( the CSA Code).

Meaford Long Term Care Centre collects, uses, discloses, and stores information about you and your health.  This information is collected to help provide health care or payments for health care.

This includes:
  • Your contact information and the contact information of your next of kin or individual authorized to act on your behalf
  • Information about your health, health care history and the health care that you have been given
  • Information about payment for your health care including your Ontario Health Card Number
We use this information and share it only with those who need to know that informaiton.

For instance, we use it to:
  • Make decisions about the types of services you need
  • Provide direct patient care
  • Communicate with other service providers
  • Monitor provision of services and evaluate your response to services provided
  • Administer services, strategic planning , quality control, research, teaching , risk management, allocate resources within the organization, and verify eligibility for services
  • Verify eligibility for payment by the Ministry of Health and Long Term Care
  • Meet Legal and regulatory requirements
Note: Including the above , if you do not object, we will:
  • Disclose your name and location to a person representing you religious organization if you provide us with information about your religious affiliation
  • Respond to inquires from family and friends confirming your presence in the facility, your room number and provide information on your general health status
THESE ARE YOUR RIGHTS;
  • You may see or have access to , or receive a copy of your personal health information
  • You may ask us to correct your patient record
  • Your personal health information is private. Unless sharing it with other is permitted or required by law, we cannot and will not give out any or your personal health information without consent.
  • You may ask questions or make a complaint about our information management practices to the Administrator.  This position, as part of its responsibilities assumes the role of Privacy Officer for Meaford Long Term Care Centre
The above statement has been developed to augment your signed Admission Agreement which provides your implied consent or that of your representative or substitute decision maker. This implied consent is in accordance with the Personal Health Information Protection Act (PHIPA). In order to withdraw this consent or any part therein, you or your representative or substitute decsion maker must do so in writing to:

Volunteer Positions

VOLUNTEERS BRING JOY TO OUR HOME!

We are seeking volunteers for the following positions:

Escort: Flexible
The weather is fine, come and have a walk with us!

Drivers: Occasional
Do you enjoy taking a little tour around town or spending an afternoon at a local garden?  Our residents really appreciate a chance to get out and have some company.

Program Assistant: Weekly
This position allows for lots of flexibility and variety.  You will be assisting residents to and from programs, helping serve refreshments, while enjoying the entertainment.

Restorative Care Assistant:  Weekly
We are so proud of our Restorative Care department and Volunteers provide much needed assistance to their growing program.  Come and learn about the benefits of our Parabath treatment, and the Walking Program.
